A year after the death of Sherlock Holmes, his friend and former partner Dr. John Watson returns to medicine as the head of a clinic focused on rare diseases and disorders. Trying to rebuild his life through difficult cases and patient care, Watson finds that the world he left behind with Holmes is not finished with him, and the past begins to intrude on his new work.
